We’re looking for a Senior mental health care navigator to join our team.

Salary £30,000 per annum

Hours of Work 35 Hours per week (worked flexibly with a requirement for evening and weekend work when required by the post).

Purpose of the Role

The Community Mental Health programme aims to strengthen personal resilience and reduce health inequalities. It provides support to people who have mental health issues to enable them to re-connect and be actively involved with their local community. The programme will focus on improving health, physical activity, education, housing and financial stability.

The Senior Mental Health Care Navigator is a brand-new role and a unique opportunity to shape support via the Community Mental Health programme. The main duties include:

  • Delivering the community mental health programme in line with contract requirements
  • Identifying the needs of service users and being accountable for delivering a high-quality service
  • Increasing levels of engagement with statutory and non-statutory agencies
  • Implementing and managing co-working strategies within the multi- disciplinary approach.

Key responsibilities

The Senior Mental Health Care Navigator (SMHCN) will work directly with our Mental Health Care Navigator. The role will focus on providing support to people with mental health to enable them to re-connect with their communities, addressing barriers such as social isolation to enable disconnection from secondary services.

The SMHCN will identify and support existing groups to be accessible and sustainable. Ensuring that our Care Navigator based in a secondary setting has a wide range of quality services that meet the needs of service users.  They will identify and support existing groups to be accessible and sustainable and help people to start new community groups where gaps have been identified and develop an Alliance of mental Health Support providers within the VCSE sector.

The post holder will be located within One Knowsley and work closely with the MH Care Navigator co-located in Merseycare secondary care services and inpatient wards.

The post holder will:

  • Work to establish an alliance of community based mental health support providers, to include VCFSE organisations who’s primary purpose is not mental health but provides support for wider determinants to enable a holistic approach to support. To employ an assertive outreach and flexible approach as and when necessary to increase levels of engagement.
  • To liaise with all appropriate statutory and non-statutory agencies as required in relation to the service user’s needs.
  • To identify risk issues and implement and evaluate risk management plans according to policy and procedure.
  • To be accountable for ensuring that within the resources at your disposal all service users receive a high-quality service.
  • Build relationships with key staff in primary and secondary care becoming part of the wider system, giving information and feedback on MHCN project
  • Be proactive in developing strong links with all local agencies to enable community based referrals, recognising what they need to be confident in the service to make appropriate referrals.
  • Work in partnership with all local agencies to raise awareness of MHCN and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care.
  • Provide referral agencies with regular updates about MHCN including training for their staff and how to access information to encourage appropriate referrals.
  • Seek regular feedback about the quality of service and impact of MHCN on referral agencies.
  • Be proactive in encouraging self-referrals and connecting with all local communities, particularly those communities that statutory agencies may find hard to reach.
